Newly Appointed Graduate Students
Enrolling in the VGABP is a simple process. Please print the Enrollment/Payment Instructions to guide you through the completion of the form. Once you have printed the Enrollment/Payment Instructions, please click on Initial Enrollment Form to provide the information required for your enrollment in the plan.
Once you click Submit on the Initial Enrollment Form, a confirmation e-mail will be sent to the appropriate campus personnel to confirm your eligibility for the plan. Once we receive verification of your eligibility, which usually takes no longer than 24 hours, we will e-mail you using the e-mail address you provide on the Initial Enrollment Form in Section 1. This e-mail will confirm your enrollment in the plan, specify the quarterly premium amounts you will owe and provide contact information for any assistance you may need while enrolled in the plan.
Once you have printed the Enrollment/Payment Instructions, please click on Initial Enrollment Form at left to provide the information required for your enrollment in the plan.
For your convenience, you may pay your premiums by credit card or debit card online, or by mailing a check. All of the instructions will be found on the Enrollment/Payment Instructions, as well as on the actual enrollment form. Please follow the instructions carefully so both your enrollment and payment of the premiums are done accurately.
