Medical PPO Buy-Up Cost Billing and Payment Process
Aetna Open Choice PPO 90/70
If you are a new enrollee and choose to buy-up to the Aetna Open Choice 90/70 Medical PPO Plan, you will have the option of paying the monthly buy-up cost with a debit card, credit card or by check. You will make your selection at the bottom of the enrollment form. Please either indicate check or debit/credit card when submitting your form.
Please note: If you are currently enrolled and are making a change to the Aetna 90/70 Buy-up Option, you will not pay during the enrollment process. You will receive an invoice via email allowing you to pay online.

Ongoing Payment of your Contributions
Your buy-up cost is due monthly. GPA will collect your first payment through the enrollment form process mentioned above. Then, all consequent monthly billings will be done via email. The email will contain a web link where you will again indicate your preferred method of payment and be taken to a page where you will make your payment. If you are paying by check, you will send in your payment to:

The payment will be due by the first of the month as payment for that month’s coverage. Failure to pay your monthly buy-up premium could result in the termination of your medical coverage.
