The PBP offers Life Insurance, Short-Term Disability and a Long-Term Disability Insurance Plan. You will be automatically enrolled in these three comprehensive plans.

Life Insurance: Group Number G000925G/Policy Number 24-0925G
The Mutual of Omaha Term Life Insurance offers a $45,000 benefit, assisting you in providing financial relief to your chosen beneficiary(ies) in the event of death. Please click on the Mutual of Omaha Life Insurance link to learn more about the other benefits offered through this insurance.
Your Summary of Coverage and Coverage Booklet are included in this section. These booklets describe your coverage in full and provide you with insurance carrier contact information. Please print and review these records, and keep them for future reference.
Short-Term Disability Insurance (STD): Group Number G000925G/Policy Number 23-0925G
The purpose of this insurance is to offer you and your family the security of knowing that if you should become disabled, replacement income is available to help you financially through the period where you are unable to work. The Short-Term Disability offers you a percentage of your pre-disability earnings during a benefit period of up to 26 weeks. Long-Term Disability Insurance (LTD): Group Number 25-0925G
LTD benefits begin at the end of the STD benefit cycle, meaning, your benefits begin after 180 days after the onset of your disabling illness or injury. The LTD benefit provides you 60% of your before-tax monthly earnings, not to exceed the plan’s maximum monthly benefit amount, less other income sources. If you become disabled prior to age 62, benefits are payable to age 65 or your Social Security Normal Retirement Age.
