Garnett-Powers & Associates is pleased to offer the Northwestern University Student Dental Plan. The new plan year begins September 1st, 2014 and ends August 31st, 2015.
Open Enrollment for the New Plan Year –
How to Enroll if you are a New Student to Northwestern University
The open enrollment period for this student dental program is July 28th – August 11th, 2014. This period allows new students to enroll for the first time if they have not previously enrolled. Please note: If you are a new student and you do not enroll during this time, you will not be eligible for the program until next year’s open enrollment for the new academic year of 2015/2016, unless you have a qualifying life event, such as marriage, birth or adoption of a child, loss of other coverage or divorce.
In order to enroll, please click on Enrollment for access to the New Student Enrollment Form Instructions as well as the New Student Enrollment Form Login where you may access the New Student Enrollment Form. The Instructions include information on how to complete the New Student Enrollment Form properly, as well as instructions on how to pay for your plan choice for the new academic year. If the form isn’t completed properly, it could delay your enrollment in your selected dental plan.
Your enrollment will not be confirmed in your plan selection until the Enrollment Form has been completed, eligibility has been verified and payment has been received by Garnett-Powers & Associates. Once your eligibility has been confirmed with the appropriate campus personnel, and payment is received, an email will be sent to the email address you have indicated on the enrollment form to confirm your enrollment in the plan. If you do not receive an email within 10 days of your enrollment, please contact Garnett-Powers & Associates.
Information for Currently Enrolled Students
If you are currently enrolled in the program and are not making any changes to your coverage, you do not need to take any action at this time. You will receive a bill after August 11th from our billing department indicating your annual premium due. Simply make sure that your payment is made no later than August 22nd in order to keep your coverage intact for the new academic year which begins September 1st, 2014 and ends August 31st, 2015. No further action is required to keep your existing dental plan in place.
If you make changes to your current enrollment during the open enrollment period, you will receive a bill after August 11th, indicating the new premium due. Your premium payment is due no later than August 22nd in order for your coverage to be effective for the new academic year.
Information for Students Who Wish to Make a Plan Change
The Open Enrollment period for the Cigna Dental Plans is July 28th – August 11th, 2014. This is the period of time where you may make changes to your current enrollment. The changes you may make are:
- Change from the Cigna Dental DHMO plan to the Cigna Dental PPO plan, or vice versa
- Add your eligible dependents if you did not previously enroll them when you first enrolled in the plan
If you do make a change, we will send you a revised invoice reflecting the new amount owed.
If you do not make any changes during the above referenced time, we will keep your current enrollment as is, as long as you make your payment for the new academic plan year no later than August 22nd, 2014. If you do not pay for your coverage, we will terminate your coverage as of August 31, 2014 and dental insurance through this program will no longer be available to you as of September 1st, 2014.
In order to make a change to your enrollment, please click on Currently Enrolled Change Form Login.
The Voluntary Cigna Dental DHMO Plan
For the new plan year, beginning September 1st, the DHMO is changing from the Cigna G1-08 plan to the Cigna G1-09 plan, offering benefit enhancements. The Cigna DHMO offers comprehensive dental coverage with no annual deductible or annual dollar maximums. Consultations,* office visits,* oral evaluations,* cleanings*(also known as 'prophylaxis') and x-rays*are covered at no-charge. If you enroll in the DHMO, you will need to choose a Primary Care Dentist. For more detailed information on this plan, please click on Voluntary DHMO Plan and Rates and then the 2014/2015 DHMO G1-09 Patient Charge Schedule.
The Voluntary Cigna Dental PPO Plan
This plan offers you the option of seeking services from an in-network or out-of-network dentist. It is important to understand that the Dental PPO Plan is not as comprehensive as the DHMO plan in that there are limits to the dollar amount of services provided on an annual basis: $1,000 in-network and $500 out-of-network. The plan does provide 100% coverage with no co-pays for in-network diagnostic and preventive services such as oral exams, cleanings and routine X-rays, however the deductible does apply to both in-network and out-of-network services. Co-insurance levels can change between in and out-of-network services. Please click on the PPO Summary of Benefits to view a schedule of benefits and services offered by this plan.
Unlike the DHMO Plan, it is not necessary to choose a Primary Care Dentist during enrollment. You choose your preferred provider when you need to seek service. This plan offers you the flexibility of seeing a dentist either in-network by choosing a dentist in the provider directory, or you may see a dentist of your choice out-of-network. Choosing a dentist within the Cigna provider directory will allow you to pay lower costs than if you seek services from a dentist not listed in the directory. Please click on Voluntary PPO Plan and Rates for more details.
* There are frequency limitations regarding diagnostic and preventive services that may be accessed during any 12 consecutive month period. Please consult the Patient Charge Schedule for further details.
