Newly Appointed Postdocs
Enrolling in the program is simple. You will make your plan selections on this web site by using the online enrollment form. The start date of your plan is your appointment date. You have 31 days from the appointment date, known as your initial period of eligibility, to perform the enrollment process. If you do not enroll during that time period, you will lose your eligibility to enroll until the next year’s open enrollment period, unless you experience a qualifying life event, such as marriage, divorce, legal separation and birth or adoption of a child.
Enrolling Your Eligible Dependents
Your spouse or civil union partner and children, including legally adopted children, are eligible for this program. If you choose to enroll your dependents, you have 31 days from your appointment start date to complete their enrollment. If you do not enroll them during that time period, they will lose eligibility for enrollment until the next year’s open enrollment period, unless you experience a qualifying life event, such as marriage, divorce, legal separation and birth or adoption of a child.

- New Hire Enrollment Instructions: Please print this document to assist you in completing the Enrollment Form. It provides step-by-step instructions for each section of the form.
- New Hire Enrollment Form: This form must be completed in order for you to enroll in your selected plans. If you waive the medical, dental and vision, you will still need to complete the form in order to provide your beneficiary information for the life insurance benefit. Certain areas of the form are required to be completed before you may hit the Submit and Create Printable Form button. These areas are marked with a .
- 2016 Postdoc Premium Contributions: The contributions required for each line of coverage are shown on this page for the medical, dental and vision plans. There have been some rate changes on the medical plans for 2016. The medical plans are increasing by 9%. There are no other rate changes for 2016. The life/AD&D insurance is covered by the university and you are automatically enrolled in this coverage. If you would like to purchase additional voluntary life insurance or long-term disability insurance, those rates per certain dollar increments are shown, linked to your age bracket.
- COBRA Information: This section contains important information regarding the Consolidated Omnibus Budget Reconciliation Act of 1985 (COBRA) pertaining to your enrollment in the Postdoctoral Fellow Benefits Program. The General Notice of COBRA Continuation Coverage Rights on the enrollment form contains language that assumes you have already enrolled in the plan(s), and is included as a section of the enrollment form.
You must click Yes on the New Hire Enrollment Form, agreeing that you have read and understand this document to allow you to submit and print your form. If you are enrolling an adult dependent, when you click Yes, you are confirming that this person has also read and understands the COBRA information included in this document. If you waive coverage for yourself, this document does not pertain to you.

As mentioned earlier above, there are two documents that must be read before you are able to submit and print your form. They are: the General Notice of COBRA Continuation Rights and the Insurance Carrier Privacy Notice. Please click Yes on the New Hire Enrollment Form after reading these documents in order to submit the form for enrollment. If you do not click Yes, you will not be able to submit the form. Please also complete Section 7, providing the beneficiary information for your life insurance benefits.
