The City of Hope Trainee and Affiliate Benefit Program offers you three medical plans from which to choose during the initial enrollment period. This section of the web site provides you with explanations of the different plans available, as well as definitions of HMO and Point of Service to better assist you in making your medical plan selections. Each of the plans offered provides a broad spectrum of benefit coverage with low copays for many of the services provided. You may choose from the following plans:
- Aetna Medical HMO Plan
- Aetna Medical Open Access Managed Choice Point of Service Plan
- Kaiser HMO Plan
Explanation of Coverage for the Aetna and Kaiser Plans
In order to review and evaluate each plan to assist you with your plan choices, please begin by clicking on Aetna Plans. Then, continue on to Kaiser HMO Plan to access links to benefit summaries for both plan years.
Please note that under the Aetna plan and Kaiser plan links, you will find two separate benefit descriptions: 1) the Summary of Benefits and Coverage and 2) the Plan Summary. The Summary of Benefits and Coverage is required to be available to you per the Patient Protection and Affordable Care Act (health care reform) to provide you with a document in a standardized format. The other document is an additional plan summary that provides even more detail on the benefits offered in each plan.
The Teladoc program offered through Aetna provides you and your enrolled family members with 24/7 access to U.S. board-certified doctors and pediatricians by phone or through online video. You can actually resolve many of your medical issues through Teladoc. Once you set up your account with Teladoc, you can start to access the nationwide network of state-licensed doctors. Summary Plan Descriptions for Kaiser and Aetna
The Summary Plan Description is a legal document that provides details about your plan benefits and coverage. It describes how to get medical care, what health services are covered and not covered, and what portion of the health care costs you will be required to pay. Please read this document carefully and refer to it whenever you require medical services. For the Kaiser and Aetna plan Summary Plan Descriptions, please click on Summary Plan Descriptions and select the appropriate booklet that represents the plan in which you are enrolled.
