These three insurance programs offer a total financial solution to protect your family and your income in the event of death or disability. A brief description of each of these three programs is offered below, with more detailed benefits outlined when you click on the specific links at left. You are automatically enrolled in all three programs, whether you choose to enroll in any of the other Trainee and Affiliate Benefit Program coverage, or choose to waive that coverage. All three programs are provided by the City of Hope and you are not responsible for any premium payment.
Life and Accidental Death and Dismemberment Insurance
The Life and Accidental Death & Dismemberment insurance (AD&D) program offered by Standard Insurance Company is a comprehensive benefit that helps you responsibly secure your financial resources. You will be automatically enrolled in the Life and Accidental Death & Dismemberment benefit. The life insurance benefit amount is $50,000, in the event of death, with an additional benefit payable for an accidental death or in the case of dismemberment.

Voluntary Supplemental Life Insurance
You have the opportunity to purchase additional life insurance for yourself. When you visit the enrollment form, you may purchase additional life insurance in $50,000 increments, up to $300,000. If you purchase more than $100,000 of additional life insurance, you will need to provide proof of insurability by completing the Medical History Statement at left. This form must be completed and submitted to Standard Insurance at the time of enrollment. Please print and complete the form, then email it to COHBP@Garnett-Powers.com. The underwriting and approval of your additional insurance in excess of $100,000 is at the discretion of Standard Insurance.
When on the enrollment form, in the supplemental life insurance section, please click on the amount you would like to purchase. The amount you will need to pay bi-weekly will populate by each increment choice.
Short-Term Disability Insurance
The Short-Term Disability Program offered by The Standard Insurance offers you the security of knowing that in the event you experience a covered disability, the disability benefit offers you a percentage of your pre-disability weekly earnings during a benefit period of up to 180 days.

This program offered by Standard Insurance provides you a way of protecting your income should you become disabled. It would begin after your Short-Term Disability income protection ends, which is 180 days of paid benefits. This disability benefit offers a percentage of your monthly pre-disability earnings.
