Aetna Dental DHMO Plan
The dental DHMO plan offers a broad range of coverage, with most preventive procedures being covered at 100%, with no copay required. Such preventive services in this category are periodic oral evaluations, cleanings and x-rays. In addition, fillings, up to four surfaces, are covered at 100% with no copay required. The DHMO requires that you choose a Primary Care Dentist for yourself and any enrolling family members. Should you require a specialist, your Primary Care Dentist will provide the referral to the appropriate specialist. You may choose a separate Primary Care Dentist for each member of your family.

Aetna Dental DPPO Plan
The dental DPPO plan offers both in-network and out-of-network benefits. You will pay less accessing the in-network benefits through a participating dentist, since these dentists have agreed to a discounted fee-for-service for the procedures listed. The deductibles are less in-network as well. This plan does offer you the ability to seek care from a dentist of your choice; however, you will pay more out of pocket. Please refer to the Provider Directory Info to learn how to choose a provider in the PPO network. You do not need to indicate this information on the enrollment form. You may choose your preferred dentist at the time you seek services.
