EyeMed Vision Care
The vision plan is provided by EyeMed Vision , a nationally recognized vision provider offering a comprehensive benefit plan at a low monthly rate. The plan allows you to access services for exams, lenses and frames every 12 months.
The EyeMed Vision Care Plan is a PPO model, allowing you the flexibility of visiting a network provider, or to choose a provider out of network. The plan offers a broad range of coverage, including exams, lenses, contact lenses and frames. In addition, it allows you to obtain your exam, lenses and frames once every 12 months. Finding a Provider in the Access Network
The EyeMed Access network includes the nation’s most popular retail locations as LensCrafters, Pearle Vision, Sears Optical and Target Optical with thousands of independent providers. Please click on www.eyemedvisioncare.com to be taken to the provider directory. Under Locate a Provider on the left side of the page, select Access as your network, then enter your zip code in the appropriate field and click Submit. A list of available providers will appear, listed by distance to your location. Please call your selected provider first to make sure they are still a member of the EyeMed Vision Care Access Network.
Once your enrollment into the EyeMed Vision Care Plan is confirmed, you will be entered into the EyeMed network system. When making an appointment with a provider, they will ask you for your name, date of birth and plan number, located on your ID card.
If you choose to obtain service from an out-of-network provider, please use the Claim Form to obtain reimbursement for your services from EyeMed. Please follow the instructions on the form to properly submit both the form and your receipts for service from the out-of-network provider.
