Garnett-Powers & Associates is pleased to offer the Visiting Scholar Injury and Sickness Insurance Plan (VSISP) for Visiting Scholars, Visiting Graduate Researchers or Non-Degree Students at UCLA. This comprehensive medical plan is offered through StudentResources, a UnitedHealthcare company specializing in university health programs. The dental plan is provided by Assurant and offers a breadth of coverage through an HMO model, as well as the discount vision plan provided by VSP Vision at no additional cost.
The VSISP web site has been developed to provide information on the plan, as well as provide simple enrollment and payment options in order to ensure enrollment in the plan. Medical Coverage Requirement for Visiting Scholars, Visiting Graduate Researchers or Non-Degree Students
As a new Visiting Scholar, Visiting Graduate Researchers or Non-Degree Students on the UCLA campus, you are required to have medical insurance, either through this program, from another insurer or provided by your home country. If you have other insurance, it must meet the requirements below to waive. If you are an international Visiting Scholar with your own coverage, that coverage must also meet these standards that meet or exceed the J-1 Visa requirements, as well as provide this same coverage to your eligible dependents. The minimum levels of coverage that must be offered through your own insurance are:
- $100,000 per person, per accident/illness in medical coverage
- An annual deductible of no more than $250
- Coinsurance that does not exceed 25%
- Medical evacuation coverage must be at least $50,000
- Repatriation coverage must be at least $25,000
- Plan includes a reasonable waiting period for pre-existing conditions
- Carrier must be at least A- rated
If your insurance meets these requirements, you may waive (decline coverage) for this plan. Please complete the appropriate sections of the Enrollment Form in order to waive the coverage. You must provide proof of insurance coverage by providing a copy of your insurance policy or schedule of benefits written in English. (If your policy is not written in English, you must provide an official translation of the insurance.) You may either scan and email your document to UCLAVSISP@Garnett-Powers.com or fax it to 949-215-2275, attention UCLA-VSISP Services. If you waive coverage for yourself and/or your eligible dependents, you will not be eligible for coverage until the New Plan Year that begins September 1, 2015, unless you experience a qualifying event, such as marriage, birth or adoption of a child, or an involuntary loss of employer coverage.
Enrolling in the Plan
In order to facilitate an efficient and timely enrollment process, the enrollment form for this program must be completed online. Please first print the Enrollment Form/Payment Instructions before completing the enrollment form. These instructions provide step-by-step information to assist you in completing the form and submitting your timely payment. If the Enrollment Form is not completed properly, it could affect your enrollment in the plans and delay your access to service.
Visiting Scholars, Visiting Graduate Researchers or Non-Degree Students will not be enrolled in the plan until the enrollment form has been completed, eligibility has been verified and payment has been received by Garnett-Powers & Associates. Once your eligibility has been confirmed with the appropriate campus personnel, and payment is received, an email will be sent to the email address provided on the enrollment form, confirming enrollment in the plan. If you do not receive this email within 10 days of your enrollment, please contact Garnett-Powers & Associates.
Important! The Dashew Center for International Students and Scholars (DCISS) and the UCLA School of Medicine Visa and Licensing Office will require J-1 visitors to provide enrollment confirmation or waiver approval from Garnett-Powers & Associates to their corresponding visa office during the mandatory check-in session. You will receive confirmation or waiver approval by email. Please print your enrollment confirmation and bring to your mandatory check-in session. For those approved for the waiver, provide your visa office with the waiver confirmation and proof of medical insurance you used to waive the VSISP plan.
